I would have answered that there can't be such a thing as one and one only jazz album that I'm really fond of, but if I stop for a minute, step back, and look all the way back to 33 years ago when hearing jazz music first "clicked" for me, I have to admit that this very first album I loved, I bought it the next day after I heard it, re-bought it when it got lost after I moved several times, and bought it again when it came out onto CD.
And this is an Eddy Louiss album simply entitled Eddy Louiss Trio that's also kown as Nardis among jazz lovers (because Miles Davis' Nardis is the first song on it).
The trio is Eddy Louiss on organ (also playing bass on the organ pedals), Rene Thomas on guitar and Kenny Clarke on drums. The songs are Nardis, Blue Tempo (R.Thomas), Hot House (Tad Dameron), No Smoking (Horace Silver), You've Changed (Bill Carey/Carl Fisher) and Don't Want Nothin' (Kenny Clarke).
After I had no more jazz in my collection, this was the first one I re-bought, and when I listen to it, I still enjoy it as much as I did back on that day in 1974.

As far as I know, there were two CD reissues of this same album, with two different artworks.

So my vote goes to Eddy Louiss Trio.
I know it's not the best known jazz album so it doesn't have too many chances to win, but I understand it's not the point. The point is to share what is our favorite jazz album, and this one could never be topped by any other for me.
Of course, there is something emotive in my take on this album, since it was the one to open my eyes (or ears for that matter) to jazz, but it is my number one jazz album , has been for over 3 decades and probably will remain for a long time to come.

Thanks for nominating this one Tom, for it is the one album I wanted to second as soon as someone would mention it here.
This is the album that, once when I was listening to it again, made me realize everything I loved the most in music, wether it was jazz, rock, country or any other genre, were the blues or blues based tracks. So this is the other jazz album that means something special to me.

So here you go, I second Kind Of Blue.
